The Incident Log module includes a set of reports that can be used to help you analyse your Incident data over a six-month (or custom) period, allowing you to gain oversight of the types and severities (etc) of Incident that have occurred at your sites. This can help you to identify:
- potential ‘problem sites’ that need further assessment;
- commonly occurring hazard types that may need further mitigation; and
- unexpected upticks in Incidents that may be indicative of new gaps or issues within your controls (etc).
Eleven reports are available within your system. Unusually within Activ, these reports are not presented on individual sub-pages, but are instead accessed using a Group By filter within a single ‘parent’ page. This filter will automatically configure the provided graph to display the relevant information. The available report options are:
- By Category
- By Type
- By Sub-Type
- By Person Type
- By Severity
- By Reportability
- By Primary Cause
- By Secondary Cause
- By Hazard Type
- By Hazard Source; and
- By Raised By

To view your Incident Log’s graphs, open the Incident Log module and click on its Reports tab. Activ will provide you with a summary of the number of Incidents that have occurred in the last six months, grouped by Incident Type.
If necessary, you can use the filters at the top of the page to:
- view one of the other report options, as listed above;
- filter the selected report by specific Incident Categories;
- filter the selected report by specific Types (if viewing the Sub-Type report) or Hazard Types (if viewing the Hazard Source report);
- filter the selected report by specific Sites, if Sites has been enabled for the module; and/or
- adjust the date range that is to be included within the report.
In addition, Activ also allows you to extract the reports and their underlying raw data from your system using the two buttons at the bottom of the page:
- PDF, which generates a PDF copy of the report that can be saved to your computer; and
- Export, which downloads a CSV file of the data to enable you to perform your own analysis.
