Incident Log – Overview

The Incident Log module of Activ is a self-contained tool allowing you to log, act on and analyse accidents, near misses and other incidents in your organisation. 

A simple, best practice form prompts you to identify root cause, take appropriate corrective/preventive action and comply with legal requirements.  You can quickly record the basic facts of an incident, assisted by configurable drop-down menus.  Optionally, the Incident Log can be more comprehensive, with the facility to add witness statements and photos, link relevant files from elsewhere in Activ, attach files from outside Activ, and raise any associated Improvement Logs.

Activ automatically maintains a complete audit trail (change log) of the instances when changes are made to an Incident Log.

At the click of a button, the full Incident Log can be converted to a PDF file, enabling it to be viewed as one document and printed and saved outside the Activ system.

You can analyse your database of incidents in the Incident Log module using the Reports, Graphs and Frequency Rates features.

The Incident Log module has five tabs:

Register – all Incident Logs are listed here.  Using the fields at the top of the page, you have the option to filter/search for an Incident Log using several criteria.  Clicking on any individual Incident Log in the Summary column on this tab opens it.  The information relating to an individual Incident Log is entered, viewed and edited on a series of tabs, whose names are self-explanatory: Details, Witnesses, Photos, Linked Files, Attached Files, Improvement Logs and Change Log.

Reports – use this to generate reports on the number of incidents matching criteria specified by you.  See Analysing Incidents – Reports and Graphs of the Number of Incidents.

Graphs – use this to view reports on the number of incidents matching criteria specified by you in graph format.  See Analysing Incidents – Reports and Graphs of the Number of Incidents.

Frequency Rates – this generates a line graph plotting the frequency rates of Reportable Accidents or Recorded Injuries.  The graph plots the Frequency Rate statistic for each of 12 months for the period specified by you.  See Analysing Incidents – Accident and Injury Frequency Rates.

Settings – this tab allows you to configure the options available in drop-down menus in several of the fields in your Incident Log form so that the Incident Log module meets the exact needs of your particular organisation.  See Configure the Incident Log Details Form via the Settings Tab.
