As standard, the Incident Log module provides you with two Frequency Rates graphs:
To view these graphs, open the Incident Log module on its Frequency Rates tab and click on the appropriate button. By default, each graph will display a graph starting from the most recent available starting date (i.e. a date twelve months in the past), but you can alter the starting date using the Starting filter at the top of the page. This will enable you view an alternate twelve-month report.
In addition, Activ also allows you to extract the graphs and their underlying raw data from your system using the two buttons at the bottom of each page:
- Save As Image, which generates a copy of the graph that can be saved to your computer; and
- Export, which downloads a CSV file of the data to enable you to perform your own analysis.
Note that, regardless of which graph you wish to view, your Incident Log module must contain the following information before it can generate Frequency Graphs for your system:
- a historic log of your company’s total monthly Working Hours;
- for each month to be included within the graph, Incident and Working Hours data for that month and the eleven preceding months; and
- for any given reporting start date, Incident and Working Hours data from that month, (ideally) the eleven preceding months, and the eleven following months.

View your ‘By Accidents’ Graph
Activ’s ‘By Accidents’ graph provides you with a visual record of your company’s Reportable Accident Frequency Rates (RAFR) for the selected Start date (month and year) and the eleven months following that date.
It is important to be aware that, unlike with other graphs within Activ, the points within this graph do not represent the total number of Reportable Accidents that have been recorded in the corresponding month. Instead, they are a statistical value that represent the number of reportable accidents that have occurred per 100,000 hours that have been worked over a twelve-month period. This means that:
- the graph itself plots the statistical RAFR values for a twelve-month period; and
- each point in the graph is itself calculated using twelve months of data.

To view your company’s RAFR graph, open the Incident Log module on its Frequency Rates tab and click on the Of Accidents button at the top of the screen. Activ will display a line graph charting your company’s RAFR values for the most recently available twelve-month period.
If necessary, you can adjust the period covered by the graph by selecting an alternate date within the Starting field, and then clicking on the Go button. Note that the resulting graph will contain the RAFR values for the selected start date and the next eleven months.

View your ‘By Injuries’ Graph
Activ’s ‘By Injuries’ graph provides you with a visual record of your company’s Recorded Injury Rate (RIR) for the selected Start date (month and year) and the eleven months following that date.
It is important to be aware that, unlike with other graphs within Activ, the points within this graph do not represent the total number of incidents that have led to injury or ill-health during the corresponding month. Instead, they are a statistical value that represent the number of injury or illness-inducing incidents that have occurred per 100,000 hours that have been worked over a twelve-month period. This means that:
- the graph itself plots the statistical RIR values for a twelve-month period; and
- each point in the graph is itself calculated using twelve months of data.
